The following forms are available for viewing and printing from your computer.
Fill the form out completely and return to the Building Department in the Municipal Complex at 6154 Mayfield Road, Mayfield Heights, OH 44124 during normal business hours (Monday through Friday, 8:00 a.m . - 5:00 p.m.) or by mail. The Building Department can be accessed from the rear of the Municipal Complex.
Make all checks payable to The City of Mayfield Heights.
