Documents needed to fill out the Civil Service Application:
Current Ohio Driver's License
Copy of Birth Certificate
Proof of Citizenship (if not born in the USA)
Copy of Diplomas or GED Certificate
2" x 2.5" Recent Photo - no paper copies
Copy of Ohio Paramedic Card (for Firefighter/Paramedic Test Only)
Selective Service # & expiration date (Selective Service numbers may be obtained by calling 847-688-6888 or by visiting the website at www.sss.gov.)
The application must be notarized
General Information
GRADES
The written portion will count for 80% of the total score and the physical ability portion will count for 20% of the total score.
ELIGIBLILITY
Placement on the eligibility list will be based on the applicant's composite score which is determined by computing 80% of the raw score on the written exam and 20% of the raw score on the physical ability exam. A composite score of at least 70% is required in order to be placed on the eligibility list. Failure to pass any of the above requirements shall be cause for the candidate to be removed from the list.
POLYGRAPH
Applicants will be required to pass a polygraph test and be fingerprinted and photographed. In addition, each applicant must pass a background investigation as to character, reputation, morals and the authenticity of the answers given to the questions on the application.
ADDITIONAL POINTS
An additional 10% will be added to a passing grade of 70% if the applicant is a full-time City of Mayfield Heights employee and 5% will be added if the applicant is a current part-time employee.
RESIDENCY
Within one year of appointment employee must live within 15 miles of city boundaries.
PHYSICAL
Applicant's height and weight must be proportional with no physical impairments. Vision must be correctable to 20/20. Prior to employment applicants who qualify on the written and agility exams and are certified by the Civil Service Commission will be scheduled for a physical examination at the expense of the City.
